Health and Wellness Center

Our Health and Wellness Center is a Federally Qualified Health Clinic that provides young people with access to comprehensive medical care.  Some of our services include pediatric and adult primary care, gynecological care, pre- and post-natal care, STD and HIV screening, mental health care, access to a nutritionist and more. We accept all Medicaid and Managed Care insurance to ensure the lowest possible cost to anyone who visits our clinic. Our Health and Wellness Center also provides referrals to partner health providers in the community for other specialized care.

The Health and Wellness Center is open Monday, Tuesday, Thursday, and Friday, 9:00am – 5:00pm, Wednesday 9:00am to 8:00pm, and Saturday 9:00am to 4:00pm.  We’re closed on the following holidays: New Years Day, Martin Luther King Day, Presidents Day, Juneteenth, Memorial Day, Independence Day, Thanksgiving Day, The day after Thanksgiving and Christmas Day. The Health Center provides language access services to patients who need them, free of charge. The Health Center receives HHS funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

Behavioral Health Services

CHNY provides comprehensive mental health services at our main site. Our team of social workers and clinicians perform assessments for young people at risk of or displaying symptoms of behavioral health issues.  We provide evidence-based and informed care for those enrolled in our services.  Clinicians, including a psychologist and psychiatrist, provide treatment individually and in group settings.

MINDs

MINDs stands for Moving In New Directions. It’s a program specifically designed for young people who have experienced homelessness. Dedicated psychologists and social workers utilize evidence-based treatment methods to provide short-term psychotherapy and crisis intervention services in individual and group settings, all through a trauma-informed lens. We utilize a holistic approach that recognizes that a collaboration between residential and mental health teams is the best way to achieve better outcomes for our young people.
